error writing to device: 'EMEDIUM' (medium error)

EMEDIUM means that the tape unit could not correctly write a block to the tape. This can be due to either the tape being bad or the drive perhaps being dirty or faulty. You  should clean the drive (sometimes even by running the cleaning tape several times throught the drive) and try repeating the job on the same tape. If the job fails again, phase out this particular tape from the backup process. If the same thing happens with other tapes, then the drive probably needs to be serviced.
